Responsibilities:
This position is responsible for continuous care initiatives for the patients. In collaboration with the management team, will provide leadership and oversight in an effort to improve health outcomes and quality of life for the patients. The typical schedule for the position is Monday – Wednesday 8-5, Thursday 8-8 and Friday, 8-1 with an occasional Saturday as needed.
Requirements:
Ideal candidate will be a FNP, PA or RN with Bachelor’s or Master’s degree in nursing, health care or public administration, or related field and a minimum of 5 year’s of experience in nursing or related profession. Valid NCDL required.
